Creation of a multiple choose to create chapters

firstly, Happy new year !!

I hope my English won't be too bad ...

Well, I need to implement in a word file a multiple selection (maybe by check box ?)

And following this selection, to create the good chapters and "Under"-chapters linked ?

Do you know if it's possible to do it ?

If I have a simple exemple maybe i could create all the boxes required to my needs
