Default How do I find and replace multiple items at once?

I have a long list (100+) of items I want my document to search for and replace such as:

Find: John Doe
Replace: John Doe, MD (77)
Find: Jane Doe
Replace: Jane Doe, MD (12)

Is there a way to find and replace my whole list of 100 or more quickly? I have word 2003.
