Default command button that generates a popup for sending email

Hello,

I hope you can help me with the following:

I would like to create a command button in a Word document.
On clicking the button, a pop up comes with 2 fields and then a submit button
To (field): the user of the Word document inputs a valid email address
From (field): this one is my email address and is pre-populated and will not change.

So, when a recipient (in the To field above) receives the email, they can then reply to the email.

Can you please point me in the right direction?
Thanks,
