I don't know what is going on with your form calculation field so just delete it and then replace it with a different field type.
Add a regular field in the last column by pressing Ctrl-F9. Then inside the {} which appear paste this text so it looks like { =(B2-2)/2 \# "0.000%" } Then lock the form and try again. In this formula, we are telling Word to use the value of Cell B2. So the Vertical acceptance formula will be almost the same but point at B3 instead.
