Cool Content control checkbox calculations


I have been searching all day to get some guidance on how to calculate average ratings on a performance evaluation form. This thread:

is the closest post that I have found that talks about what I need. Your help would be so greatly appreciated. My question is pretty much the same as in the linked thread.

Each box in each section represents a score 1 -5 (1 being worst, 5 being best)
I would like to take the value for the checked box in each element and add them together then divide them by the number of factors to come up with an average score to be displayed at the bottom of each factor with a grand total at the bottom.

the first factor has 5 elements which score needs to averaged on its own and than added to the remaining factors. I hope that makes sense :-)

I would also like to prevent the user from checking more than one box per section?

I have never done this before so detailed instructions would be great if possible, please. I do not know how to work with Macros or how to write code so I am hoping this will be fairly easy :-)

Any help would be greatly appreciated :-)

Hi allikat,

You refer to scores for each 'factor', but some of them (e.g. Communication, Decision-Making/Judgment, Appearance & Grooming) don't have anywhere for the score to be output. Please clarify.

Hi - thank you so much for your help

There are 8 performance factors in total. Professionalism is the only performance factor that has 5 elements. We would like the form to calculate an average score based on the five elements and give an rating just for Professionalism.

Communication
Decision-Making/Judgment
Appearance and Grooming
Attendance
Dependability

Then take the score for Professionalism and as managers rate each performance factor, the form will give an average based on the ratings selected at the bottom of each factor and then give a grand total for all 8 factors at the bottom of the form.

Professionalism
Attention to Detail
Attitude
Ownership
Responsiveness
Teamwork & Cooperation
Ethics
Basic Job Knowledge

It's not the number of elements I'm concerned about, but with where the output for each factor is to go. The factors I mentioned don't have an output row in their tables.

Try the attached. If you save it as a macro-enabled template, any new document you create from it will have access to the code without having to be saved in the docm format - provided the template remains accessible to the document.
