Hi Bird Fat,
Thanks for your response. I do not have the 'view > toolbars > forms option on the version of Word that I am using (Microsoft Office 2003). Do you know how to bring up the Forms option with this version? I hope you respond asap, as my boss is piling on the pressure for this template to be completed bu the end of today!

It IS there - as I aimed the instructions at that version!
If you open the View menu - click on the little double chevron icon at the bottom of the list to expand it - Toolbars is the 7th one down, directly below Task Pane.
