Default ICONS change to a different icon

Truly, I'm not crazy, but sometime an icon on the task bar changes to another icon -- in my cases, the icon gets changed to the one for Adobe Illustrator. However, if you click on it, it points to the right place. This has happened to me three times.



The first time, it showed the AI icon for the PS icon. I made sure my original AI icon worked correctly, so I deleted the new one (I've often found that duplicates cause trouble). However this deleted EVERYTHING about Photoshop CS6, which I'd spent hour installing the previous day. Reinstalled PSCS6, saw the PS icon, went and did something else, returned to my desktop and saw that the PS icon had changed to the AI icon again. I realized that the PS icon was NOT a shortcut,but the real thing, so when I deleted it, t deleted the the entire program and all files. So I changed the PS icon to be a shortcut (and changed the AI icon as well.

Then today I see another AI icon on the toolbar. This time it's pointing to a fie I'd been working on earlier. Where can I go to report this to Microsoft?

Sorry, I forgot to saythat I'm running Windows 8.1 on a Dell 64-bit machine.
