Customer questionnaire

I would like to know if it is possible to make a customer questionnaire with Powerpoint Basically I would like a yes/no system whereby the information is stored and then the file is returned to me for viewing the results Obviously I would like a field with the customers name . Very ambitious I know for a novice but is there any tutorials out there for me to see how it is done Many Thanks Dean
