Default Customer questionnaire

Hi
I would like to know if it is possible to make a customer questionnaire with Powerpoint Basically I would like a yes/no system whereby the information is stored and then the file is returned to me for viewing the results Obviously I would like a field with the customers name . Very ambitious I know for a novice but is there any tutorials out there for me to see how it is done Many Thanks Dean

Hi
I am not sure why you want to use PowerPoint for your questionnaire. It would be better to use MS Word and design an online form. You can create text boxes and combo boxes for multiple choice selections.
There is plenty of help on the web about creating online forms in MS Word.

otpm Many thanks for the reply That's interesting I never would have thought of using it One last thing I have searched the web including You Tube none of them mention a web based form and how it gets forwarded or viewed by the originator which would be me Any idea's Thanks Dean

Hi Dean
You did not mention you wanted a web based form. However if your company is using Sharepoint then the form could be accessed via the web. This would seem the best way forward.
