Default Simulate a log in screen

A few years ago, while I was with a differnet company, the task assigned was to address an issue of operators not logging out of a terminal and performing tasks under another users name.



This week I was assigned a new task, see above.

For the life of me I cannot remember how I did it.

A dialog box appeared requiring criteria below.
User Trainer
Password ******* (real pass word was Boston1)

If either was incorrect the fields would clear.

Once the correct information was entered the dialog box would disappear.

Any help on this would be greatly appreciated.
