Default Customising Outlook Appointments

Hi,

Is there a way in which, whenever I open a new appointment in Outlook, I can have a document / form appear automatically in the main body of the appointment?

The purpose of this would be for someone running a business to take bookings over the phone. The idea is that when they take a call, they merely click on new appointment and a table would automatically appear in the main body containing a template table which I have created with all the information headings required for that booking.

I have gone through the process of creating a form, but to open the form several steps are required (File, new, choose form, personal forms library, open) which is not ideal if you're trying to take a phone booking at the same time! Even when I "customise commands", I still have at least 3 steps to go through.What i'm looking for is a quicker way of having this form open up either automatically whenever I select "new appointment" or at least in as few steps as possible!
