Default Outlook 2010 email setup, return address

Trying to find a way to setup my email address in Outlook 2010 to indicate ONLY my name and email address as being at gmail.com. The basic setup in Outlook only provides that my local ISP be shown, how to setup so that only my gmail address is shown? See JPG image attached.

To help you with your request, can you answer some troubleshoot questions?
- Within your Outlook client, do oyu have a Gmail account configured?
- If you do not, please create a pop or imap account connecting to your gmail account and selected it a the primary account to be used. When this has been completed does it show ou as only the gmail address when sending?
