Default Two Different Names in the Same File

I need to put two different names living at the same address on one address card. i.e. John Doe and Mary Smith live at 1234 Main.

When I make mailing labels, I get John Doe & Mary Smith on the label, which is what I want. The problem is with the form letter. I get Dear John, and NO Mary.

How do I format the address card so I get Dear John and Mary on the greeting when I mail merge to a forn letter?

Any help will be appreciated.
