Not sure this is the right place for this question.

1. I am looking to create a radio button on a custom form I created in outlook. I can't figure out why the multi line is greyed out. The user should be able to choose between Cash, Check or Credit Card.

2. While I created the user defined fields I can't figure out the formats. For the credit card I want to create a field where the format of the card number would be 9999 9999 9999 9999 and the exp date would be mm/yy. Where and how do I put that info?

There are some issues that you might want to consider here. Presumably this 'form' is something that you propose to send out via e-mail for others to complete.

You have no control over how your message will be viewed by the recipient, so any element you add to a message is likely to be incompatible with the receipient's mail software.

Who in their right mind is going to send you credit card information in a format as insecure as e-mail?

ok. here is the background in a nutshell. First my sincere apologies because I really was very unclear.

I have an educational hotline for married couples. I am using an outlook custom form as a database to enter their information. The custom form I am creating has nothing to do with email. It is simply a way I can save the information they give me over the phone. This form is not being sent or being received through email.

When a person wants to subscribe to the hotline they call me with their name, address and cc info. While I enter the info into the fields I created the exp field is acting funny probably because I created it as a date and there is no "day" part. In addition the radio button comes with only 1 choice and multi-line is greyed out.

Can this be accomplished it outlook?
