Default fill in form + forwarding to another email

Hi everyone.

I really need some help with the particular process from Outlook. So basically it must to look like this:
1.I receive the email from the user
2.Than the user must to fill it in active form (PDF, Word Document or something else) . So some fields must be fill in to save and process the document.
3.When the person fill the form than automatically it is send to another email address
So is it possible to create somehow the rules in outlook that could check if the form is fill in and if it is than send this form to another email address?

Maybe it is some software or add-in for outlook to do this. What about active form how can I make them?
It would contain basic information as name, surname, phone number, and so on.

For any help I will be very appreciate.

Best regards

Lucas
