Default Custom .fdm forms cannot be sent - value required

Sometimes I need to use custom forms in my job, and one of those does not work - I put information in all required fields, but still there is the following error message appearing twice: A field on this form requires a value.



I think the form might be a reworked version of an older form. I have a developer tab on, but not really sure how to go about this. Any ideas?
