Default Using Outlook Form on Forwarding email messages

Hi all,

Firstly I wanted to say what a great forum this is and so full of great information. I am a newbie to the site so apologies if this question has already been asked.

A team at work have a team inbox, I have a new process whereby they will monitor the inbox for relevant emails coming into the team. I want them to forward various emails they receive to another department but using a Outlook form with 3 or 4 data entry fields populated with instructions.

I have created the Outlook Form type (Note) with all the relevant fields, now is the point where I am struggling. When a user opens a relevant email, I want them to be able to open the form, fill in the various fields and send on to the other department with the form and original email. The form itself will contain supplementary data to the original email.

At the moment I have the form as a shortcut on the outlook toolbar via a Macro, any help on this will be invaluable.

Thanks in advance
