Default Forms in Outlook

I would love to create an e-mail template in outlook used to take written notes on phone called that people missed while away from their desks.



For example the outlook message would be an .oft which would have non-editable field labels names like: "Name", "Phone Number", and "Project". These would be right beside editable fields corresponding to their labels. This way, one could tab down the the form entering all the necessary information.

Can this be achieve easily in outlook?

You may decide a template with (editable) labels saved in the body would be simpler.
