Default Filling in email from form

Hi

I don't know if this is possible but what I'd like to do is use a custom form to fill in several (intentionally) blank spaces in an email template with data input by the user on the form.
I can create a form that will email the details from the form but not how to get it to do what I describe?

Any advice would be great.

Will the people who receive the form all be connected to the same Exchange server? IF so, you can follow these instructions to build a form: http://office.microsoft.com/en-us/ou...001210610.aspx

Otherwise, you can use a standard Outlook template (which is just an email that has been pre-filled out by you - but users can edit any section or otherwise edit the responses any way they choose).
