Default How to make an email message 'form'

Please excuse the vagueness of this question, but I want to do something in Outlook, and my research to date has thrown up a confusing number of options. All I am looking for is some guidance on the right way to approach this, then I can go and investigate.

I want to create an 'Email Template', which is a form for people to fill in. The information carried in this form will be a series of questions, which require either a written comment, or a pull-down selection. There will also need to be some text boxes to fill in.



Forms looked like being what I wanted, but from what I have seen so far this is more geared towards customising office. I may have this wrong.

A template sounds more like what I want, but I am not sure how to included the structure of the form with text boxes and pull-downs.

Any guidance, or steering that can be given to point me in the right direction would be gratefully received.
