Default How to distribute and retrieve information with Outlook 2003 form

I'm sure this has been asked (and answered) a 1,000 times. My apologies for asking it once more. I need to send a simple project status form to a staff of about 5 people in my department. I want to use an Outlook 2003 form to do it. I want to be able to email it to this small 5-name distribution list, have them complete the form and reply to my email with their answers. I've tried creating a public folder and publishing my form to it. Testing it has produced one failure after another. I know I'm missing a step (or two), but I can't figure out what those are. Your wisdom would be appreciated.
