Default Outlook 2007 form checkbox lockdown

Hello,

I want tocreate a very simple form in Outlook 2007 (from the Standard Forms Library -meeting request ). I want to modify the P.2 and add 7 checkboxes. each box will be named as an equipment in confrence room (projector, TV, DVD player...). Requestor needs to check what equipment she/he needs by ticking the box. Problem is when the receiver of this request receives this reservation request can uncheck or check the boxes. I want the requestor to be able to disable (or lock) the choice she/he made. I have no programing background. I would appreciate your help.
