Default Receive error cannot open this form because an error occurred in BCM 2007

I receive the following error message when I try to create a new marketing campaign in BCM 2007: "cannot open this form because an error occurred"

I've tried:
1. Creating a new profile
2. Clearing the forms cache (deleted the forms folder)
3. Updating both Outlook and BCM
4. Repair installation
5. Uninstall and then reinstall of Office 2007
6. Deleted the forms folder again
7. increased the size of forms cache
8. tried http://support.microsoft.com/kb/919596/



any ideas?
