Default Adding controls to a form.

I am working on making a custom form, for purchase requisitions, however, the controls do not show up, on the final form. I thought they may be behind the table, this form has no controls, but one i tried did not work.

I need to add a "Submit for Approval" button, which would email it to a particular person, for approval, and then display the button as an "Accept" button.

Any ideas would be helpful, Thanks...
