Default Printing and Saving Outlook Forms

I am trying to create a form in Outlook for Public use, which has been created successfully. However, when the end user tries to save and print the form, all the lables, fields and drop down options disappear. I've checked the properties on all the fields and the tick box to include the field for Printing and Save As is selected, yet none of the fields are observing this fact.

Could someone tell me if there is something else I should be doing please. Thank you.
