Default Creating phone message template

I am trying to create a custom phone message template in OneNote 2010. A routine task is to collect voice mail messages left while our office is closed. There are often 5-10 of these. I want to have a OneNote notebook phone message section (got that already), with one page for each workday. For each message, I want to have a table with data labels (time and date of message, originating phone number, name of caller, message, callback number, possibly a "done" checkbox) in the left column, with the data corresponding to each label in the right column.

I know how to create this table. What I don't know is how to set a custom toolbar button that will paste a new copy of it for each message. It seems like there ought to be a way of doing this. Does anyone have any suggestions, or in the alternative, a strategy for a workaround?

There is a OneNote template (actually, a couple of them) for phone messages, but it creates one page for each message. Not what I have in mind.
