The data in your Word document would need some pre-processing before it would be suitable for use as a mailmerge data source, in Word or Excel.
Firstly, all the data for each record should be on one row - you have it spread out over multiple lines/paragraphs Secondly, you would need to have the same number of fields per record - some of your records have more lines (fields) than others. Thirdly, you'd need a header row to tell the mailmerge what each field represents.

Getting each block of data into a single line is the easy part. The difficult part is going to be in getting it aligned properly, so that you can end up with all the fields properly separated. Some have two email addresses, others have one, those with two email addresses will have everything after the first email address out of alignment with those that have only one email address. And that's without knowing whether the physical addresses all habe the same number of lines. You would do better to get the data output from whatever system generated it, with all the fields properly aligned and delineated.
