Paul has missed the steps - set the table document as a Label merge document. Cancel the dialog to select a label that pops up, and then follow the steps he indicates after 'Next, format the first label...'

You might have difficulty with the custom paper size if your printer will not print to that size. Use a standard paper size and margins to make up the difference.
