Default Help with VLOOKUP

I am relatively new to excel.

I have a worksheet that I need to populate a column based on two variables.

If I have entered Name in one row, and entered New or Old into next, I would like the third row to automatically populate with the numerical information (as entered in a seperate table as shown below)

Name New Old
John 11 111


Fred 22 222
Paul 33 333
Ian 44 444
Chris 55 555
Andy 66 666

I have tried VLOOKUP and it will only llo at one column of data.
How do I go about doing this please?

Perhaps you wish CONCATENATE, which will create an automatic combination of various values.
