Default Concatenate text and a date

Hi all

I am trying to concatenate the following, but I can't get my formula to work correctly.

In cell AC3 - the words "Installation"
In cell AF3 - is the installation date
In cell AH3 - the words " Warranty Expires"
In cell AK3 - is the expiration date

Can someone please help me with the correct formula as my dates are appearing as numbers.

Many thanks
