Default Excel Formula for Multiple Conditions & Results

Hi, I need some help on Excel formula that I'm currently working on. The formula is working except for the 1st line (attached). Correct coding/result is "patient-janitorial", but current formula comes back as "patient". I don't know what to add or change the formula to give me the correct result. Thank you for your help!

Your formula is rather complicated.

However, I don't see any string like "patient-janitorial" in it, so it cannot be returned

You are correct, I haven't added any string since I didn't know how and what to add to make the current formula return "patient-janitorial". Do you have any suggestion?

Could you please explain ( briefly but in plain English) what the conditions are to get one string or another so that we can understand the logic?

Thank you

The first portion of the IF statement (logical_test) returns "Uni" (value_if_true) for certain item number, description, or location. And the rest (value_if_false) will return "mammo" if the item number is "MDT~", otherwise it returns "patient".

I thought I can add a string in the (value_if_false) that will return "patient-janitorial" if the item number starts with "D", but I wasn't successful.

Please let me know if there are any other info needed. Thank you for your help!
