Add prefix to .csv from excel

I'm creating a large file of contacts in excel which will be saved as .csv and imported to my contacts in gmail. That's done. I've named and number figured out, but I can't figure out how to add a prefix.

I've tried "prefix", "name prefix", and "prefix name" and under all those variations it shows up in the notes of the contact rather than filling in the prefix field.
