Can anyone please help me with the attached document.

I want to create a front data entry screen that will populate the sheet behind it. An example of it is attached below.

The Notebook number will be scanned in on the front page. Ill then fill in the rest of the data like name, id number, dates etc. This info should all carry over to page two next to the corresponding notebook number.
Then when i re scan on page one the notebook number all information relating to that notebook will show on screen one as well....

Any help would be great.
