Default Printable Entry Form


I'd like to create some sort of entry form which I can use to enter data to be saved in a separate sheet or workbook. I'd like the form to be printable. I have a couple of sheets of data. One is an address list. The other is an inventory list. I'd like to be able to use data from those sheets in the form as well as send data from the form to the sheets. I'd like to be able to use the form as a printable document as an invoice. I would appreciate any input.

This is a rather vague question.. perhaps elaborate some more.
