Default Anyone willing to do a good deed?

G'day. I'm Excel illiterate & I need a simple (?) table put together. No formulas are needed. Just need this jpeg of the original created again so we can change numbers when needed.

Sorry to ask for this favor but to attempt it is sadly distressing. I'm afraid I'm stuck at the shallow end of the pool with MS Word.

Hope someone can help out.



P.S. It for our local seniors exercise & social group.

Default Form in Word

I do not work too much in Excel but, do work in Word often. If you can use the form in Word (in table format), it is attached.
