I don't know how to read the .frm file you attached so I have no idea as to the specifics of what that should look like.

Excel does have an automatic 'form' interface that might work for you if you didn't need a lot of customisation. See this video for an example of how you might make it work

If you wanted a VBA Userform which is highly customisable but will take a long time to create then have a look at this video to get an understanding of what will be involved.
