Default Populate sheet 3 with data from sheet 1 and sheet 2

Hello all,
Please review my attached workbook.
I am trying to have sheet 3 populate the data from sheet 1 and sheet 2 when the command button is pressed. However it is not working like I need it to, please see sheet 3 after pressing the command button. I need it to look like sheet 4.
Any help.
Ohh and I need this to all work based on the "File Number" not the employees name.

I hope someone can help.
