Default Organization Chart Object Changes to Equation Object?

Hello all,

Flowcharts were pasted in a document as organization chart objects.
Right-clicking on the object, dialogue window showed it as “Organization Chart object”.
Double-clicking opened it in the Organization Chart Add-in for editing.

There are also equations in the document, entered with equation editor.
Right-click on those, and they are shown as “Equation Object”
Double-clicking opens it in the equation editor for editing.

At some point during revisions and re-savings of the document, the organization chart object becomes classified as an “equation object”.
Right-clicking, the dialogue box will show it as an equation object.
Double-clicking will open for editing, but it will not open as the flowchart, it will open as the equation.
Closing the equation window will replace the flowchart with the equation, and I have to “undo” to get the flowchart back.

Any ideas on why this happens and how to prevent?

Thanks.
