Picture Content Control Place Holder Duplicating

Hi all. Pardon if this was asked elsewhere, but I'm having trouble with Picture content controls in a template I've created.

Essentially, I have created a template using quick parts for drop-downs to select the number and placement of Picture content controls so that the folks and my office can quickly add standard images to our documentation.



However, sometimes after the picture is added, the blue "insert picture" boxes will seem to repopulate themselves so both the inserted picture and the new "insert picture" box will display.

I have been unable to get the problem to happen consistently so I am a bit stuck on trouble shooting. Any word from anyone who has seen a similar issue would be greatly appreciated.
