Your problem is caused by having autoshapes for underlining, instead of using proper underlining. On the whole, the document is poorly constructed, with autoshapes for underlining and creating boxes, table cells that aren't wide enough for data entry, and so on.

Your document will require a lot of work to set right. I suggest you hire someone who is skilled in document design to do this correctly. In the meantime, here is a macro you can use to move all the lines that are presently behind the tables to the front, so that you can then select and delete them.
Code:
Sub MoveToFront() Dim Shp As Shape For Each Shp In ActiveDocument.Shapes If Shp.WrapFormat.Type = wdWrapBehind Then Shp.WrapFormat.Type = wdWrapFront End If Next End Sub

Without more details, I cannot possibly diagnose that issue. You could simplify the macro, though, to explcitly force all shapes 'in front', not only those that are 'behind':
Code:
Sub MoveToFront() Dim Shp As Shape For Each Shp In ActiveDocument.Shapes Shp.WrapFormat.Type = wdWrapFront Next End Sub

The line that was previously in the middle of the telephone line has gone. The line that is now at the bottom of that line is actually a cell border applied to the tops of the cells below the telephone line.
This goes back to what I said before about your document's poor design. The lines are just not done with any consistency or in a way that facilitiates maintenance.
