If you're creating the form from a template, you could use a CREATEDATE field to output the date the form was first saved. The only way to change that date is by re-saving the file via Save As. If the user doesn't know the date is created via a CREATEDATE field (which can't usually be discerned in a document using 'filling in forms' protection), they're unlikely to circumvent it via Save As. Of course, if you don't use a CREATEDATE field to output the date on the form, the user may well remain unaware that the creation date can be extracted at any time (e.g. via a macro) ...
