Try using:
"<([0-9]{1,2})(/)([0-9]{1,2})(/)([0-9]{2})>"
The < and > characters tell Word the found characters must start and end a word, respectively. If you want to include both 2-digit and 4-digit years, use:
"<([0-9]{1,2})(/)([0-9]{1,2})(/)([0-9]{2,4})>"
