Hand written and computer forms do not go hand in hand well.

I would suggest using a two column table, with the second column having a bottom dotted border. A fixed row height of .61 cm should work. It will look OK when printed out and it lends itself better to automatic data extraction when the form is computer filled.

I had hoped to add an attachment, but I have a major internet failure here caused by a line fault. It should be fixed soon, but in the meantime uploading files is a problem.

PS Those nice fellows from the telecoms company have just been and fixed the line, so I have uploaded the example file.
