Outlook 13 unwanted attachment image

I have Outlook 13 with Windows 8.1 OS. When viewing my 'Sent' folder in Outlook my email show an attachment clip when no attachment was sent. How do I prevent that clip image from appearing next to me Email address when no attachments were set?
