Legacy formfields are NOT content controls. This is a different tool. I assume you are not using Content Controls but rather legacy formfields.

Here is a follow up on the StyleRef Field, which should be the tool you need. You would create a character style (not a linked or paragraph style) to mark the information you want to be in the header.
