Default mail merge multiple rows within a grouping

I have template that I am trying to mail merge data from an excel spreadsheet. I can't seem to get directory to work. I want grouped by provider with all the members under the provider listed in the same table. I have attached the sample template and sample excel files.

If the table goes on more than one page I need the heading repeated. I want a new page everytime the provider changes.
