Unhappy Multiple page numbering

Hi,

My question is this. I have an extremely large document (500+ pages) that consists of Chapters, Annexes and Appendices and I wish to number the pages as follows:

Title page (i)
Contents page (ii) onwards (Should be an automatically updated TOC)
Amendments page (follow on from last contents page in same format)

Chapters:

1-1first number is chapter number second is page number.

Annexes:

1-A-1 first number is chapter number, second is annex letter third is page number of Annex

Appendices:

1-A-1-1 first number is chapter number, second is annex letter third is appendix number fourth page number of Appendix

For example 2-B-3-2 would be:

Chapter 2
Annex B
Appendix 3
Page 2


The page numbers would be held in the page footer. I have followed the instructions here for customised page numbers and have got that to work but can't figue out how to add the other levels.

The document will be created in word 2003
