Hello all i hope you can help as i dont know the method about creating this document.

Well I want to create an document where i can select pre written text that will add it into a document like;-

"Part 1"
Part 2
Part 3
Part 4
Part 5

But to add this pre-written text with a form and radio buttons, im sure this is possible, i just dont know the way to go about it.

ps i m not a student =)
