Question Form auto insert

I am trying to create a form which can be auto filled in.

I want to have a list of titles each with a checkbox in column A, when the check box is selected a word file needs to be insterted in column B.

For Example
Model 1 Selected insert file with specification for model 1

Final document should only have specifications for the items selected.
