It would be helpful if you actually had the content control checkboxes in your document, preferably in the paragraphs to which they relate.

The same applies to the content control dropdown list for which you want to vary the formatting (plus a clear description of what those variations are.

As for your 'field to calculate operations', you might say where, exactly that is to go and what specific inputs are used for this. Likewise, you should include the 'status icon/figure' for the conditional colouring.
